In 2019, I worked a WMS go-live with a 3PL warehouse in Ontario, California - 80,000 square feet, 12,000 SKUs. First week after cutover, the mispick rate jumped from 2% to 9%. Customer service got buried in calls. The software was fine. The integrations were fine. Three days of digging later, the implementation team found the real problem: the data migrated from the legacy system. More than 800 SKUs had no barcode at all, 300-plus had messed-up units of measure (cases and eaches mixed together), and 50-some location codes didn't match between the old and new systems. The owner spent $400,000 on a new system and got taken down by a handful of spreadsheets.
